Referral Officer – Johannesburg, Braamfontein

 

Community Media Trust (CMT) is a South African NGO focusing on communication in the fields of basic human rights. CMT aims to empower individuals and communities, encouraging them to take part in managing and improving their own lives, promoting healthy and health-seeking behaviour and basic human rights. It does this through media and outreach programmes that provide scientific information in a format that is easy to understand.

An exciting opportunity exists for a qualified community referral officer to participate in a ground-breaking HIV prevention programme focusing on counselling and Testing (HTS) in the greater Gauteng area as CMT seeks to employ a community referral officer who will be responsible for strengthening linkages between facilitators and HIV Testing Services (HTS) as well as other health and social services.

Key duties and responsibilities:
(may include but are not limited to)

  • Strengthen linkages between the multi-session facilitators and HIV Testing Services (HTS) as well as other health and social services.
  • Work to ensure that the referral loop is closed and that the referral is completed satisfactorily.
  • Work closely with DOH and other stakeholders to establish community-based structures that will create demand for HTS and for treatment, care and support
  • Coordinate activities with identified health facilities to ensure that clients referred from structured programmes are provided with the continuum of care within a health facility.
  • Report weekly to the coordinator on tracking and tracing of referrals and completing the referral registers.
  • Work closely with the M&E department to ensure that the data is collated, submitted and verified.
  • Comply with ad-hoc requests from your supervisor

VMMC Communications Coordinator, Umhlanga, KZN

 

Community Media Trust (CMT), a national NGO that focuses on empowering individuals and communities to take part in managing and improving their health through media and outreach programmes is looking to recruit a committed, enthusiastic and socially-minded Voluntary Medical Male Circumcision (VMMC) Communications Coordinator to work in Umhlanga, KwaZulu-Natal.

Applicants should enjoy working as part of a team and have initiative to engage partners and community members in VMMC Communications activations.

 

Responsibilities may include, but is not limited to the following:

  • Following through on leads generated from mass media. Making sure client is booked and then circumcised. This information is to be entered onto a spreadsheet
  • Assisting with follow up calls
  • Assisting with co-ordination and planning of focus group testing
  • Keeping track of weekly community activations, activities and events and covering these for use on social media
  • Informing mass media lead clients of any changes in schedules or services
  • Generating social media content from the field
  • Liaising with local media including newspaper and radio to generate organic press
  • Co-ordinating community events and engaging all relevant stakeholders
  • Engaging with other NGO’s CBO’s and stakeholders to include CMT campaign work
  • Ensuring accurate use of consistent branding and placement at events
  • Recording of all demand generation activities at community level

REMUNERATION:  R12,000.00 per month

Community Mobilisers – Benoni, Ekurhuleni

CMT seeks to employ 2 committed, enthusiastic and socially-minded community mobilisers (CMs) who would be based in Ekurhuleni and would be able to start on immediately.

The Community Mobilisers will be expected to network with potential support and partner organisations locally to plan and execute mobilisation activities, such as open day and door-to-door events that will inform and educate communities while assisting to improve the demand for health services related to HIV prevention.

CMs will report to the Ekurhuleni Project Manager. This post requires provincial travel and the availability to work some weekends to fulfill the project goals.

  • Develop and oversee monitoring systems to track success of communications programme implementation
  • Design research and evaluation efforts to assess the impact of VMMC Communications Campaigns
  • Develop, monitor and update an activity tracker and ensure the implementation thereof
  • Conduct and oversee daily analysis and tracking of communication activities and ensure data collection systems are in place and functional
  • Ensure accurate recording and management of collected data
  • Prepare and present Communications Analysis and narrative reports
  • Manage a team of VMMC Communications Coordinators
  • Engage with other NGO’s, partners and stakeholders
